Mataya is a member of the Ely community and works at Essentia Health-Ely Clinic as a Pathways to Wellness Care Facilitator. She connects people to resources supports and needs like insurance, housing, transportation, social supports and mental health services. Care Facilitation Services are FREE.

Heidi Vanderbeek

"I have been in the care facilitation role here at St. Anthony's for almost 5 years. It's a humbling and magnificent experience to witness the positive changes in a person's life once they are given the chance to thrive and succeed.  Many people need help on a personal level which I truly believe God is calling me to assist with. To meet an individual where they are at by partnering and advocating for their needs. I can help with navigating insurance, social services, transportation, safety, food access-the scope is wide. I work with all community members, families and parishioners. Everyone deserves to be happy, healthy and accepted."

Kate Willis

"I am Kate Willis, I have been connecting people with resources for five years as a Care Facilitator with Wellbeing Development (Pathways to Wellness.) I have a Community Health Worker Certificate from Northeast Technical College in Bemidji, Minnesota. I have worked in Home Care with the Ely Hospital and also have worked in nursing as a Licensed Practical Nurse. I have learned a lot as a Care Facilitator. One valuable asset is patience because it takes time to navigate the social services and many other systems that connect people. We can help you find the various services that may help you to achieve your goals to improve your life."
Family Resource Facilitator

Ryan Stewart

Ryan works for Ely Community Resource. Prior to working for ECR, Ryan worked at The Voyageur Outward Bound school where he led wilderness expeditions aimed at improving the lives of at-risk youth and their families. His work background has been focused on utilizing the wilderness as a place to build social and emotional resilience. He has a Master’s in Environmental Education from UMD. His studies focused on the impacts of Outdoor Education on individual’s abilities to set and accomplish their personal goals. He utilizes the helping skills learned at Outward Bound to help families and youth find access to local resources.
